Description:The Registered Nurse, in the Anticoagulation Clinic, serves the patient, the physician and other staff members by assisting in the delivery of primary health care and patient care management. This individual also works to develop, interprets, implement and document a plan of care for each patient.
Duties:(This list may not include all the duties assigned.)
1. Elicits critical patient information about anticoagulation status and factors potentially affecting anticoagulation status.
2. Analyzes patient information and laboratory data.
3. Selects and follows appropriate Warfarin dosing protocol, upon orders from the Medical Director.
4. Recognizes patient circumstances and lab data which require referral and communications to anticoagulation Medical Director or PCP.
5. Triages patient problems during clinic visit and over the telephone.
6. Assumes responsibility for arranging patient follow up on identified problems.
7. Determines frequency of patient visits based on patient information and lab test rewults.
8. Initiates contact with appropriate home health agencies to arrange patient care at the approval of attending anticoagulation Medical Director or PCP.
9. Analyzes home care situations for potential risks to an anticoagulated patient.
10. Watches for and recognizes musculoskeletal hematomas and injuries; notifies attending anticoagulation Medical Director, PCP or emergency room as appropriate.
11. Watches for and recognizes symptoms of GI or GU bleeding and initiates appropriate referrals, based on perceived urgency.
12. Watches for and recognizes thrombosis or embolus and contacts PCP, attending anticoagulation Medical Director or emergency room, as needed.
13. Recognizes unusual responses to Warfarin dose regulation and contacts attending anticoagulation Medical Director or PCP.
14. Teaches patients and families about Warfarin risks, benefits and precautions.
15. Engages patient in appropriate self-care activities concerning anticoagulation use.
16. Records all prothrombin time results, anticoagulation dose, return visits and telelphone interactions in clinic chart. Sends copy of results to PCP in timely manner or at intervals requested by PCP.
17. Documents all prothrombin time results and resultant Warfarin dosage using Anticoagulation Clinic computer program. Sends copy of results to PCP in timely manner or at intervals requested by PCP.
18. Participates in professional development efforts to ensure currency in health care practices and trends.
19. Maintains strictest confidentiality
20. Attends meetings as required
Qualifications:PERFORMANCE REQUIREMENTS:
Knowledge, Skills & Abilities: Skill in communicating effectively and empathetically with patients and their family members; Ability to work effectively with providers and staff; Specialized knowledge in the area of counseling and related services; Knowledge of community counseling resources; Ability to develop and maintain a resource library for patients; Knowledge of computer systems and applications; Knowledge of health care administrative practices; Ability to build relationships with internal and external departments and agencies; Knowledge of administrative principles and the ability to exercise initiative, problem-solving and decision-making.

Education: RN Required, BSN preferred

Experience: Two to three years of clinical health care experience with at least one year of supervisory experience.

Certificate/License: Current Oregon RN license. Healthcare provider CPR required.
